Important Information About Required Annual Immunization Reporting

The Department of Children, Youth & Families (DCYF) is partnering with the Washington State Department of Health (DOH) to help early learning and child care providers navigate submitting their annual immunization status report this year.

Schools, preschools and child care providers must complete and file an immunization status report to DOH each year per WAC 246-105-060.

While submitting the annual report is not a new requirement, there were changes to immunization requirements this year. Due to these changes, the deadline is extended to February 1, 2020, for this year only. Reporting officially opens on December 2, 2019.

Reporting is required for:

  • Licensed child care centers with 13 or more children
  • Preschools located at a public or private school
  • Head Start and ECEAP programs

Reporting is optional for:

  • Licensed family home child care providers with 12 or fewer children
  • Private preschools not located at a public or private school
  • Co-Op preschools

Reporting will be conducted via SurveyMonkey. PDFs and paper reports will not be accepted this year. Providers will receive more information in November with instructions and resources for submitting reports on December 2.
